Welcome New Patients!

Welcome to our dental family! At Parsell Family Dentistry, we understand that your time is valuable and want to make your every visit as easy as possible. If this is your first time visiting our office you will need to complete a few documents, so that we can start your dental records.  This registration typically takes 5-15 minutes to complete, and can be done from home if you would like. In order to help us get your appointment started on time, please download our new patient registration forms below and complete them prior to your appointment. Bring them with you at the time of your visit and we will scan them into your records and get your appointment started.
